Abstract
Metal ceramic restoration 은 cast metal의 margin의 정확성과 강도를 ceramic의 심미성과 합친 장점을 가진 형태의 수복물로 과거 30년간 전치부의 심미적인 보철을 위한 소재로 각광 받고 많이 사용되어 왔다. 하지만 metal collar에 의한 비심미적인 부분을 제거하기 위하여 많은 술자들은 치아삭제 시finishing line을 subgingival level에 형성하게 되었고, 이것에 의한 많은 문제들이 보철 치료 후에 나타나게 되었다. Fluid control의 실패와 연조직 손상에 의해 정확한 인상채득이 어렵게 되었고 이로 인하여, 치경부의 black shadow 뿐만 아니라 gingival inflammation이 일어나게 되었다. 이러한 문제들을 해결하기 위해서는 정확한 finishing line의 형성과 임시 보철물에 의한 심미수복이 필요하다.
As patients have concerned the esthetics of dental restorations, the porcelain fused to metal restorations have gain their popularity due to their strength and esthetic appearance for many years. However, metal collar in subgingival level of porcelain fused to metal restoration often causes black shadows. Inadequately positioned metal collar causes plaque depositions, gingival inflammations, black shadows, and discolorations of margin area. Those problems can be avoided by a clear finishing line, well-fabricated provisional restorations with precise margin and fine polishing, collarless porcelain fused to metal restoration, and all ceramic restoration.
